I believe it is Chinese in origin. The Chinese character for 4 sounds similar to the character for death. As a result, Chinese people believe it is bad luck, especially for hospitals, to have a 4th floor. It's the same type of superstition that some buildings in the US do not have a 13th floor.

Funny how its bad luck to have a fourth floor in hospitals, but its merely a matter of convenience to have a funeral home on the premises as the vast majority of hospitals do...
